{"data":{"id":"us-ky/krs-304.17-030","jurisdiction":"us-ky","citation":"KRS 304.17-030","heading":"Scope and format of policy.","body":"No policy of health insurance shall be delivered or issued for delivery to any person in\nthis state unless it otherwise complies with this title, and complies with the following:\n(1) The entire money and other considerations therefor shall be expressed therein;\n(2) The time when the insurance takes effect and terminates shall be expressed therein;\n(3) It shall purport to insure only one (1) person, except that a policy may insure,\noriginally or by subsequent amendment, upon the application of an adult member of\na family, who shall be deemed the policyholder, any two (2) or more eligible\nmembers of that family, including husband, wife, unmarried dependent children to\nage nineteen (19), unmarried children from nineteen (19) to twenty -five (25) years\nof age who  are full -time students enrolled in and attending an accredited\neducational institution and who are primarily dependent on the policyholder for\nmaintenance and support, and any other person dependent upon the policyholder as\nprovided pursuant to KRS 304.17-310;\n(4) The style, arrangement, and overall appearance of the policy shall give no undue\nprominence to any portion of the text, and every printed portion of the text of the\npolicy and of any indorsements or attached papers shall be plainly printed in lig ht-\nfaced type of a style in general use, the size of which shall be uniform and not less\nthan ten (10) point with a lower case unspaced alphabet length not less than one\nhundred and twenty (120) point (the \"text\" shall include all printed matter except\nthe name and address of the insurer, name on title of the policy, the brief\ndescription, if any, and captions and subcaptions);\n(5) The exceptions and reductions of indemnity shall be set forth in the policy and other\nthan those contained in KRS 304.17 -050 to 304.17-290, inclusive, shall be printed,\nat the insurer's option, either included with the benefit provision to which they\napply, or under an appropriate caption such as \"Exceptions,\" or \"Exceptions and\nReductions,\" except that if an exception or reductio n specifically applies only to a\nparticular benefit of the policy, a statement of the exception or reduction shall be\nincluded with the benefit provision to which it applies;\n(6) Each form, including riders and indorsements, shall be identified by a form n umber\nin the lower left-hand corner of the first page thereof; and\n(7) The policy shall contain no provision purporting to make any portion of the charter,\nrules, constitution, or bylaws of the insurer a part of the policy unless the portion is\nset forth in full in the policy, except in the case of the incorporation of, or reference\nto, a statement of rates or classification of risks, or short -rate table filed with the\ncommissioner.","path":[],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=17288","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:57:45Z","sha256":"1f2ce5da9fbad33d5ffef4742cb408b32e59669a171674b141c19157c081e7bf","source_id":"us-ky","stale":false,"prev":"us-ky/krs-304.17-020","next":"us-ky/krs-304.17-040"},"notice":"GroundRules: Original legal text.
