{"data":{"id":"us-ky/krs-304.17a-262","jurisdiction":"us-ky","citation":"KRS 304.17A-262","heading":"Coverage for orchiectomy or orchidectomy as treatment for","body":"urological cancers.\n(1) All insurers issuing or renewing a health insurance policy, certificate, plan, or\ncontract, including but not limited to a health benefit plan, th at provides coverage\nfor orchiectomy or orchidectomy as treatment for testicular or other urological\ncancer shall provide coverage, in a manner determined in consultation with the\nattending physician and the insured patient and subject to applicable cost s haring\nthat is consistent with the cost sharing established for other benefits under the\ncoverage, for the following:\n(a) All stages of surgical reconstruction related to the orchiectomy or\norchidectomy, including testicular or other urological prostheses; and\n(b) Physical complications of all stages of orchiectomy or orchidectomy,\nincluding any related surgical reconstruction.\n(2) An insurer shall provide written notice to an insured of the availability of coverage\nfor an orchiectomy or orchidectomy upon enrollment and annually thereafter.","path":[],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=54273","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:57:47Z","sha256":"d67fd9f44627df973028c10a84111ccf68825d077ef65a663c6babed45eecfa7","source_id":"us-ky","stale":false,"prev":"us-ky/krs-304.17a-261","next":"us-ky/krs-304.17a-263"},"notice":"GroundRules: Original legal text.
