{"data":{"id":"us-ky/krs-304.17a-330","jurisdiction":"us-ky","citation":"KRS 304.17A-330","heading":"Self-insurance reporting requirements -- Exemption.","body":"(1) All insurers authorized to write health insurance in this state and employer -\norganized associations that self -insure shall transmit at least annually by July 31 to\nthe commissioner the following information, in a format prescribed by the\ncommissioner, on their insurance experience in this state for the preceding calendar\nyear:\n(a) Total premium by product type and market segment;\n(b) Total enrollment by product type and market segment;\n(c) Total cost of medical claims filed by product type and market segment;\n(d) Total amount of medical claims paid by the insurer and insured by product\ntype and market segment;\n(e) Total policies canceled by type and the aggregate reasons therefor; and\n(f) List of total health and medical services paid for, grouped by types o f services\nand costs:\n1. Total cost per health and medical service per insured group:\na. Cost paid by insurer;\nb. Cost paid by insured; and\n2.
