{"data":{"id":"us-ky/krs-304.17a-718","jurisdiction":"us-ky","citation":"KRS 304.17A-718","heading":"Disclosure of claims payment information to covered person.","body":"(1) Beginning on January 1, 2001, upon issuance, delivery, or renewal of a health\nbenefit plan in Kentucky, an insurer shall:\n(a) Clearly indicate on each covered person's identification card the mailing\naddress where a claim for payment shall be sent; and\n(b) Issue new identification cards or an appropriate sticker to covered persons no\nlater than thirty (30) calendar days following the effective date of any change\nin the address of the insurer, its agent, designee, or other entity that processes\nclaims for the insurer.\n(2) Identification cards for covered persons shall identify whether the covered person\nhas health maintenance organization (HMO), point of service (POS), preferred\nprovider organization (PPO), or indemnity fee for service (FFS) coverage.","path":[],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=29338","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:57:48Z","sha256":"2652dacc6be7cd6075fe6c72f2417b3c089d73669ff6426b3c451018608dba21","source_id":"us-ky","stale":false,"prev":"us-ky/krs-304.17a-716","next":"us-ky/krs-304.17a-720"},"notice":"GroundRules: Original legal text.
