{"data":{"id":"us-ky/krs-304.17a-838","jurisdiction":"us-ky","citation":"KRS 304.17A-838","heading":"Members to receive evidences of coverage -- Contents.","body":"(1) Every member of a self-insured employer-organized association group shall receive\nwritten evidence of coverage by the group.\n(2) All evidences of coverage issued pursuant to this section shall contain coverage\nterms, conditions, and exclusions.\n(3) All evidences of coverage issued pursuant to this section shall contain the following\ndisclosure in prominent, contrasting type: THIS COVERAGE HAS BEEN\nPLACED WITH A SELF-INSURED EMPLOYER-ORGANIZED ASSOCIATION\nGROUP WHICH HAS RECEIVED A CERTIFICATE OF FILING FROM  THE\nCOMMONWEALTH OF KENTUCKY.
