{"data":{"id":"us-ky/krs-304.18-030","jurisdiction":"us-ky","citation":"KRS 304.18-030","heading":"Required provisions in group policies.","body":"Each such group health insurance policy shall contain in substance the following\nprovisions:\n(1) A provision that, in the absence of fraud, all statements made by applicants or the\npolicyholders or by an insured person shall be deemed representations and n ot\nwarranties, and that no statement made for the purpose of effecting insurance shall\nvoid such insurance or reduce benefits unless contained in a written instrument\nsigned by the policyholder or the insured person, a copy of which has been\nfurnished to such policyholder or to such person or his beneficiary.\n(2) A provision that the insurer will furnish to the policyholder for delivery to each\nemployee or member of the insured group a statement in summary form of the\nessential features of the insurance cov erage of such employee or member and to\nwhom benefits thereunder are payable.
