{"data":{"id":"us-ky/krs-304.47-030","jurisdiction":"us-ky","citation":"KRS 304.47-030","heading":"Required statements for applications and claims forms.","body":"(1) All applications shall contain a statement in a form approved by the Department of\nInsurance that clearly states in substance the following: \"Any person who\nknowingly and with intent to defraud any insurance company or other person files\nan application for insurance containing any materially false information or conceals,\nfor the purpose of misleading, information concerning any fact material thereto\ncommits a fraudulent insurance act, which is a crime.\"\n(2) All claim forms shall contain a statement in a fo rm approved by the Department of\nInsurance that clearly states in substance the following: \"Any person who\nknowingly and with intent to defraud any insurance company or other person files a\nstatement of claim containing any materially false information or conceals, for the\npurpose of misleading, information concerning any fact material thereto commits a\nfraudulent insurance act, which is a crime.\"\n(3) Fraud warning statements shall not be required on applications or claim forms used\nby reinsurers.","path":[],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=30184","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:57:59Z","sha256":"f6a20d82c5f371717ae9faa27fc57e663c78f34045198457730c49b74bf9b06b","source_id":"us-ky","stale":false,"prev":"us-ky/krs-304.47-025","next":"us-ky/krs-304.47-040"},"notice":"GroundRules: Original legal text.
