{"data":{"id":"us-ky/krs-311.580","jurisdiction":"us-ky","citation":"KRS 311.580","heading":"Forms of licenses and permits.","body":"(1) Every certificate of regular license, limited license, fellowship training license,\nprovisional permit, and temporary permit shall be in such form as is approved by\nthe board or by the executive director thereof; provided, however, that each\ncertificate sh all be signed by the executive director and shall contain the date of\nissuance, the name of the person to whom issued, and whether the holder is\nauthorized to practice medicine or osteopathy.\n(2) Limited license and fellowship training certificates, whethe r originals or renewals,\nshall expressly state the period therefor, which shall not be in excess of one (1) year,\nand shall expressly state the nature and place of work authorized thereunder.\n(3) Temporary permits shall expressly state the date of issuance , that they shall be for a\nperiod of not more than six (6) months from date of issuance, and that they are\nsubject to cancellation as provided by KRS 311.575.","path":["KRS Chapter 311"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=30528","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:58:04Z","sha256":"858df536e002e6a0bd8a080d68c34b42b3fef55c38b4053710857b18b347eb27","source_id":"us-ky","stale":false,"prev":"us-ky/krs-311.575","next":"us-ky/krs-311.585"},"notice":"GroundRules: Original legal text.
