{"data":{"id":"us-ky/krs-315.4102","jurisdiction":"us-ky","citation":"KRS 315.4102","heading":"License required for each facility of a third-party logistics provider.","body":"(1) Each  facility of a third-party logistics provider located within Kentucky shall be\nlicensed by the board prior to shipping a prescription drug:\n(a) Within the borders of Kentucky; or\n(b) To a location outside the borders of Kentucky.\n(2) Licenses issued under subsection (1) of this section shall be renewed annually\nupon:\n(a) Completion of an application; and\n(b) Payment  of a renewal fee as established by administrative regulations\npromulgated by the board.\n(3) A  third-party logistics provider located in another state seeking to ship a\nprescription drug into Kentucky shall provide documentation upon request by\nthe by the board or its staff that the third-party logistics provider is licensed as a\nthird-party logistics provider by:\n(a) The  state from which the third-party logistics provider ships, if that state\nlicenses third-party logistics providers; or\n(b) The United States Food and Drug Administration.\n(4) A  third-party logistics provider license shall be valid only for the name,\nownership, and location listed on the license. Changes of name, ownership, or\nlocation shall require a new third-party logistics provider license.\n(5) Changes  in information required for licensure shall be reported to the board, in\nwriting, within ten (10) days of the change.\n(6) A third-party logistics provider shall not operate from a place of residence.\n(7) A third-party logistics provider facility shall be located apart and separate from\nany retail pharmacy licensed by the board.\n(8) A  third-party logistics provider shall publicly display all licenses and have the\nmost recent state and federal inspection reports readily available.","path":["KRS Chapter 315"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=46698","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:58:12Z","sha256":"157d0acb4592cd01a4fa1e6cd858da6c83afcc858bb053936830df8995203875","source_id":"us-ky","stale":false,"prev":"us-ky/krs-315.410","next":"us-ky/krs-315.4104"},"notice":"GroundRules: Original legal text.
