{"data":{"id":"us-ky/krs-319b.050","jurisdiction":"us-ky","citation":"KRS 319B.050","heading":"Contents of license -- Display -- Application for replacement -- Name","body":"change.\n(1) Any license issued by the board shall contain the name of the person to whom it is\nissued, the address of the person, the date and number of the license, and other\ninformation the board deems necessary. The address contained on the license shall\nbe the address where all correspondence and renewal forms from the board shall be\nsent. Any person whose address changes shall, within thirty (30) days after the\naddress change, notify the board of the address change. The most recent address\ncontained in the board's records for each licensee shall be the address deemed\nsufficient for purposes of service of process.\n(2) Every licensee shall either keep his or her license prominentl y displayed in the\noffice or place in which the licensee practices or have it stored in a place from\nwhich it can be immediately produced upon request of a patient or a representative\nof the board.\n(3) Any person whose license has been lost or destroyed ma y apply to the board for a\nreplacement. This application shall be accompanied by an affidavit setting out the\nfacts concerning the loss or destruction of the original license and the payment of a\nreasonable replacement fee as established by the board in KRS 319B.030(1)(f).\n(4) Any person whose name is changed by marriage or court order or who changes\nemployment, home address, or telephone shall notify the board in writing within\nthirty (30) days of the change.","path":["KRS Chapter 319B"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=31281","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:58:15Z","sha256":"9753230bd927b5b666322bf0931dae4909ab14064d023e5112ab7ff8a9160577","source_id":"us-ky","stale":false,"prev":"us-ky/krs-319b.040","next":"us-ky/krs-319b.060"},"notice":"GroundRules: Original legal text.
