(Effective January 1, 2028)\n(1) Each licensing authority listed in subsection (4) of this section shall collect the\nfollowing data using a data collection app lication, during the annual, biennial, or\ntriennial licensure renewal process:\n(a) Licensee's personal information, including but not limited to:\n1. The licensee's first, middle, and last name;\n2. Date of birth;\n3. Mailing address, email address, and phone number;\n4. Physical address of all practice locations;\n5. License type;\n6. License number and date of issuance;\n7. License expiration; and\n8. National Provider Identifier number; and\n(b) Workforce participation information, including but not limited to:\n1. Degree level or certificates completed;\n2. Employment type;\n3. Employment specialty;\n4. Employment setting;\n5. Employment status, including but not limited to:\na. Actively employed in respective field;\nb. Actively employed in a clinical setting, either in person or through\ntelehealth, or both;\nc. Actively employed outside of respective field;\nd. Unemployed but seeking work in respective field; or\ne. Unemployed but not seeking work in respective field;\n6. Reason for unemployment, if applicable; and\n7. Ability to meet patient needs of those covered under the federal\nAmericans with Disabilities Act.\n(2) Data collected pursuant to subsection (1) of this section shall not include a\nlicensee's Social Security number.\n(3) Licensees may notify the licensing authority if the licensee wishes to decline the\nuse of data collected under subsection (1)(a) of this section to be made available to\nother employers or agencies for free or for purchase.\n(4) The following licensing authorities are subject to the requirements of this section:\n(a) Professional licensure boards as defined in KRS 211.332;\n(b) Kentucky Board for Medical Imaging and Radiation Therapy established in\nKRS 311B.030;\n(c) State Board of Podiatry established in KRS 311.410; and\n(d) Kentucky Department for Public Health, in regard to community health\nworkers certified under KRS 309.462.\n(5) Each licensing authority listed in subsection (4) of this section shall report as\nnecessary, and upon re quest, the data collected under subsection (1) of this section\nto the Office of the Kentucky Center for Statistics established in KRS 151B.132 and\nthe Cabinet for Health and Family Services for workforce capacity and workforce\nreporting and tracking purposes.","path":["KRS Chapter 335B"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=58324","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:58:25Z","sha256":"f239ff49169ca2c40d2726a486637c4887bf2a9db29174f37f3de760338f33c0","source_id":"us-ky","stale":false,"prev":"us-ky/krs-335b.070","next":"us-ky/krs-335b.080"},"notice":"GroundRules: Original legal text.
