{"data":{"id":"us-ky/krs-367.624","jurisdiction":"us-ky","citation":"KRS 367.624","heading":"Contractor's duty to disclose cancellation rights and provide standard","body":"form.\nPrior to entering into a real estate goods or services contract with any person, a contractor\nshall furnish the person with:\n(1) The mailing address of the con tractor through which written communication may\nbe received;\n(2) The telephone number of the contractor and, if applicable, the contractor's facsimile\nnumber and email address;\n(3) The following notices in at least ten (10) point boldface type:\n(a) \"You ma y cancel this contract at any time before midnight on the fifth\nbusiness day after you have received written notification from the insurer that\nall or any part of the contracted goods, services, or goods and services is not a\ncovered loss under the propert y, casualty, or property and casualty insurance\npolicy. This right to cancel is in addition to any other rights of cancellation\nyou may have under state or federal law or regulation. See the attached Notice\nof Cancellation form for an explanation of this right.\"; and\n(b) \"This contract shall not assign or otherwise transfer, in whole or in part, your\nduties, rights, or benefits under the property, casualty, or property and\ncasualty insurance policy in violation of KRS 304.20 -105. Any contract\nentered in vio lation of KRS 304.20 -105 shall be void and unenforceable.\";\nand\n(4) A fully completed form in duplicate, under the conspicuous caption \"NOTICE OF\nCANCELLATION,\" and attached to but easily detachable from the contract, in at\nleast ten (10) point boldface type that shall read as follows:\n\"NOTICE OF CANCELLATION\n.......................................................\n(enter date of transaction)\nIf you are notified by the insurer that all or any part of the contracted goods,\nservices, or goods and services is not a covered loss under the property, casualty, or\nproperty and casualty insurance policy, you may cancel this contract without\npenalty or monetary obligation before midnight of the fifth business day after you\nhave received the notice.
