{"data":{"id":"us-ky/krs-367.681","jurisdiction":"us-ky","citation":"KRS 367.681","heading":"Contents of contact lens prescription -- Contents of prescription for visual","body":"aid glasses -- Refusal to release prescription prohibited.\n(1) A contact lens prescription shall include the following:\n(a) The ophthalmic information necessary to acc urately fabricate or dispense the\nlenses, including the lens manufacturer, lens series, and the lens material if\napplicable;\n(b) Power and base curve;\n(c) Name, license number, telephone number, and for written orders, the signature\nof the prescribing optometrist, osteopath, or physician;\n(d) Patient's name and address, expiration date of the prescription, and number of\nrefills or lenses permitted; and\n(e) The date of issuance.\n(2) A contact lens prescription may also include the diameter, axis, add power, cylinder,\nperipheral curve, optical zone, and center thickness.\n(3) A prescription for visual aid glasses shall include the following:\n(a) The name, license number, telephone number, and for written orders, the\nsignature of the prescribing optometrist, osteopath, or physician;\n(b) The patient's name;\n(c) The date of issuance; and\n(d) The value of all parameters the licensed optometrist, osteopath, or physician\nhas deemed necessary to dispense corrective lenses appropriate for a patient.\n(4) A licensed optom etrist, osteopath, or physician shall not refuse to release a\nprescription for contact lenses or visual aid glasses to a patient.","path":["KRS Chapter 367"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=47636","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:59:05Z","sha256":"18669b3df1427b3793fd3f471b7bad00637aa2f7eb551da42dfe830b927b2f71","source_id":"us-ky","stale":false,"prev":"us-ky/krs-367.6802","next":"us-ky/krs-367.682"},"notice":"GroundRules: Original legal text.
