{"data":{"id":"us-ky/krs-402.100","jurisdiction":"us-ky","citation":"KRS 402.100","heading":"Marriage license -- Marriage certificate -- Confidentiality of Social","body":"Security or other government-issued identification card numbers.\nEach  county clerk shall make  available to the public the form prescribed by the\nDepartment for Libraries and Archives for the issuance of a marriage license. The\ndepartment shall issue a marriage license form which provides for the entering of\ninformation identifying each party as a \"bride,\" \"groom,\" or \"spouse.\" This form shall\nprovide for the entering of all of the information required in this section, and may also\nprovide for the entering of additional information prescribed by the Department for\nLibraries and Archives. The form shall consist of:\n(1) A marriage license which provides for the entering of:\n(a) An authorization statement for any person or religious society authorized\nto perform marriage ceremonies to unite in marriage the parties named;\n(b) Vital information for each party, including the full name, date of birth,\nplace of birth, race, gender, condition (single, widowed, or divorced),\nnumber  of previous marriages, occupation, current residence, relationship\nto the other party, and full names of parents;\n(c) A  statement signed by both parties swearing that, to the best of their\nknowledge, the information provided on the form is correct; and\n(d) The date and place the application was made.\n(2) A marriage certificate which provides for the entering of:\n(a) A statement by the person performing the marriage ceremony or the clerk\nof the religious society authorized to solemnize the marriage ceremony\nthat the ceremony was performed. The statement shall include the name\nand  title of the person performing the ceremony or the name  of the\nreligious society solemnizing the marriage, the names of persons married,\nthe date and place of the marriage, and the names of two (2) witnesses;\n(b) A statement by the person performing the marriage ceremony of his legal\nqualification under this chapter to perform the ceremony, such statement\nto include the name  of the county or city where his license to perform\nmarriage ceremonies was issued or, in the case of religious societies\nauthorized by KRS 402.050(1)(c) to solemnize marriages, the name of the\ncity or county where the religious society is incorporated. The provisions\nof this paragraph shall not be construed to require the clerk of a religious\nsociety to be present at the marriage so long as the witnesses of the\nsociety are present;\n(c) The  printed name  and dated signature of the person performing the\nceremony; and\n(d) A signed statement indicating that the marriage certificate was recorded\nin the county in which it was filed. The statement shall also include the\ntitle of the county clerk or deputy clerk of the county in which the\ncertificate was filed and indicate the date the marriage certificate was\nrecorded.\n(3) A certificate to be delivered by the person performing the marriage ceremony\nor the clerk of the religious society performing the marriage ceremony to the\nparties married. This certificate shall provide for the entering of:\n(a) A statement by the person performing the marriage ceremony or the clerk\nof the religious society performing the marriage ceremony  that the\nceremony  was performed. The statement shall include the name and title\nof the person performing the ceremony, or the name  of the religious\nsociety performing the ceremony, the names of persons married, the date\nand place of the marriage, and the names of two (2) witnesses; and\n(b) A dated signature of the person performing the ceremony or the clerk of\nthe religious society performing the ceremony.\n(4) A Social Security card or other government-issued identification card shall be\nrequested as a means of identification of each party but the number shall not\nbe recorded or retained.","path":["KRS Chapter 402"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=47035","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:59:28Z","sha256":"7082e9f92d5ed6fd946c87ccabeac78085a85b384731638c0b9e4598bdf4685f","source_id":"us-ky","stale":false,"prev":"us-ky/krs-402.090","next":"us-ky/krs-402.105"},"notice":"GroundRules: Original legal text.
