{"data":{"id":"us-ky/krs-41.375","jurisdiction":"us-ky","citation":"KRS 41.375","heading":"Issuance of duplicate check -- Liability for loss sustained by issuance.","body":"(1) A duplicate check shall be issued by the Treasurer upon:\n(a) Receipt of a notarized affidavit, which may be transmitted to the Treasury in\nan electronic format, signed by the payee and stating that the original check\nhas been lost or stolen; and\n(b) A review of the records of the Treasury, including the unified and integrated\nsystem of accounts to determine that the original check has not been presented\nfor payment.\n(2) The payee receiving a duplicate check shall make good any loss the Commonwealth\nor the Treasurer may sustain on account of the issuance of the duplicate or the\npresentation and payment of the original, if the loss has been caused by actions of\nthe payee.
