The following fees shall be collected in advance by the secretary of state for health maintenance organization:\n(1) For filing a charter, other documents, and\namendments thereto, per page\t$ 2.50\n(2) For certified copies of any document,\nper page\t$ 2.00\nB. The following fees and licenses shall be collected in advance by the commissioner of insurance for health maintenance organizations:\n(1) Certificates of authority\t$ 500.00\n(2) Repealed by Acts 2012, No. 206, §1.\n(3) (a) For filing annual statement\t250.00\n(b) Penalty for unauthorized late filing\t1,000.00\n(4) Initial examination of health maintenance\norganization companies\t1,000.00\n(5) Repealed by Acts 2012, No. 206, §1.\n(6), (7) Repealed by Acts 1996, 1st Ex.
