The board shall assess and collect fees as follows:\n(1)\nFee for education review and temporary\n$ 25.00\npermit\n(2)\nFee for renewal of temporary permit\n$ 25.00\n(3)\nFee for reinstatement of delinquent\n$ 15.00\ntemporary renewal\n(4)\nPenalty for late payment of renewal of\n$ 15.00\ntemporary permit fee\n(5)\nFee for duplicate license\n$ 20.00\n(6)\nFee for reinstatement of suspended\n$100.00\nor revoked license (in addition to all past\nunpaid renewal fees)\n(7)\nPenalty for late payment of renewal\n$ 15.00\nof license\n(8)\nFee for reactivation of license\n$100.00\n(9)\nFee for reexamination\n$ 25.00\nB. The fees established in this Section shall not be refundable except under such conditions as the board may establish.","path":["TITLE 37.
