The following fees shall be assessed by the board:\n(1) For each examination given by the examination team to an applicant:\n(a) Theory examination\t$25.00\n(b) Practical examination\t$25.00\n(c) Re-take of any examination\t$25.00\n(2) For each initial certificate of registration and each annual renewal of such certificate:\n(a) Cosmetologist, esthetician, manicurist, teacher/instructor, or manager $35.00\n(b) Beauty shop or salon, mobile salon, or home care\t$55.00\n(c) Initial inspection fee for salon, mobile salon, or booth rental\t$50.00\n(d) Booth rental permit, special permit, or temporary permit\t$50.00\n(3) If a registrant wishes to restore an expired certificate listed in Paragraph (2) of this Subsection within three years from the date of expiration, he shall be assessed a fee equal to twice the applicable fee for each year the certificate was expired, not to exceed three hundred dollars.\n(4) Student registration including one \"in-state\" transfer\n(a) Resident\t$10.00 upon entering the course\n(b) Nonresident\t$10.00 upon entering the course\n(5) Initial certificate of registration for a school\t$340.00\n(6)(a) Renewal of certificate of registration for a school\t$190.00\n(b) Failure to timely renew\t$175.00\n(7) If a registrant wishes to restore an expired certificate listed in Paragraph (5) of this Subsection within three years from the date of expiration, he shall be assessed a fee equal to twice the renewal fee for each year the certificate was expired in addition to the fee for failure to timely renew.\n(8) Other school fees:\n(a) Initial inspection\t$100.00\n(b) Final inspection\t$300.00\n(c) Change of ownership notice\t$ 50.00\n(d) Change of address notice\t$100.00\n(9) Miscellaneous fees:\n(a) Official certification of records\t$15.00\n(b) Duplicate license\t$25.00\n(c) Copy of booth rental contract form\t$ 5.00\nB. The board shall pay a per diem of one hundred dollars to each board member and each examination team member.","path":["TITLE 37.
