The board is authorized, by rule promulgated in accordance with the Administrative Procedure Act, to establish fees and costs which shall be within the range prescribed by the following schedule:\n(1) License fees:\tMinimum Maximum\n(a)License by examination$ 85.00$200.00\n(b) License by endorsement\t50.00\t100.00\n(c) Duplicate license\t20.00\t30.00\n(d) Renewal of license\t30.00\t60.00\n(e) Reinstatement of license\nwhich has been suspended,\nrevoked, or has lapsed by\nnonrenewal\t100.00\t300.00\n(f) Duplicate renewal\t10.00\t20.00\n(g) Delinquency fee in\t50.00\t100.00\naddition to renewal fee\nfor nursing license\n(per year delinquent)\n(2) Survey fee\t250.00\t500.00\n(3) Renewal of certificate\nof accreditation\t100.00\t200.00\n(4)Evaluation of credits:\n(a) Applicants for admission\nto approved program\t25.00\t50.00\n(b) Out-of-state applicants for\nLouisiana practical nurse\nlicense\t50.00\t100.00\n(5) Repealed by Acts 2018, No. 162, §1.\n(6) Miscellaneous fees and costs\n(a) Photocopies of documents\nper page\t.50\t1.00\n(b) Certificate of document\nas true copy\t5.00\t10.00\n(c) Repealed by Acts 2018, No. 162, §1.\n(d) Handling and mailing per page\t1.00\t2.00\nB.
