These circumstances shall include all of the following:\n(1) Time of birth.\n(2) Date of birth, including the year, the month, and the day.\n(3) Place of birth, address, including city or town and state; if in a hospital or other institution, its name.\n(4) Whether a plural birth. A separate certificate is required for each child in a plural birth.\n(5) If a plural birth, the number of each child in order of birth.","path":["TITLE 40.
