Each employee to whom this chapter applies shall furnish the treasurer of the governmental unit, in such form as he shall prescribe, such information as is necessary to enroll himself alone, or himself and his dependents, under the policy or policies of insurance.\nEach employee shall receive a certificate or booklet from the insurance company, savings bank or non-profit hospital, medical, or other service corporation describing the coverage for which the employee has enrolled.
