The application for a license to operate a commercial vehicle or for a learner's permit to operate a commercial vehicle shall include the following:\n(1) the full name and current mailing and residential addresses of the applicant;\n(2) a physical description of the applicant, including sex, height, weight and eye and hair color;\n(3) date of birth of the applicant;\n(4) the applicant's Social Security number;\n(5) the signature of the applicant;\n(6) a photograph of the applicant;\n(7) certifications, including those required by 49 CFR 383.71 (a);\n(8) consent of the applicant to release driving record information; and\n(9) any other information required by the registrar.\nThe application shall be accompanied by the application fee required by the provisions of section thirty-three of chapter ninety.\nWhen a licensee changes his name, mailing address or residence, an application for a duplicate license shall be made forthwith to the registrar.\nNo person who has been a resident of the commonwealth for thirty days or more may operate a commercial motor vehicle under the authority of a license to operate a commercial motor vehicle issued by another jurisdiction.","path":["Part I","Title XIV","Chapter 90F"],"source_url":"https://malegislature.gov/Laws/GeneralLaws/PartI/TitleXIV/Chapter90F/Section7","current_through":"amendments passed before May 31, 2026","vintage":"","retrieved_at":"2026-09-17T05:21:49Z","sha256":"8229d6b2614027d35eb5d4f74196b707c72c5514b9f739a17962b727ca58bd71","source_id":"us-ma","stale":false,"prev":"us-ma/mass.-gen.-laws-ch.-90f-6","next":"us-ma/mass.-gen.-laws-ch.-90f-8"},"notice":"GroundRules: Original legal text.
