Code, Health - General § 10–615","heading":"","body":"Each application for involuntary admission to a facility or Veterans’ Administration hospital under this part shall:\n(1) Be in writing;\n(2) Be dated;\n(3) Be on the form required by:\n(i) The Administration, in the case of a facility; or\n(ii) The Veterans’ Administration hospital, in the case of a Veterans’ Administration hospital;\n(4) State the relationship of the applicant to the individual for whom admission is sought;\n(5) Be signed by the applicant;\n(6) Be accompanied by the certificates of:\n(i) 1 physician and 1 psychologist;\n(ii) 2 physicians;\n(iii) 1 physician and 1 psychiatric nurse practitioner;\n(iv) 1 physician and 1 licensed certified social worker–clinical; or\n(v) 1 physician and 1 licensed clinical professional counselor; and\n(7) Contain any other information that the Administration requires.","path":["Article - Health - General"],"source_url":"https://mgaleg.maryland.gov/mgawebsite/Laws/StatuteText?article=ghg\u0026section=10-615","current_through":"2026-01-01","vintage":"","retrieved_at":"2026-09-14T19:58:48Z","sha256":"c9d44e6c0e2f0c0a8a68a6ad0b71c74b3731d12c5843c818b4c844d7164b81a0","source_id":"us-md","stale":false,"prev":"us-md/md.-code-health-general-10-614","next":"us-md/md.-code-health-general-10-616"},"notice":"GroundRules: Original legal text.
