Code, Health - General § 19–1B–02","heading":"","body":"(a) There is a Community Integrated Medical Home Program.\n(b) The mission of the Community Integrated Medical Home Program is to:\n(1) Keep Maryland families healthy through the use of innovative mapping tools that allow better targeting of resources to those in need;\n(2) Coordinate comprehensive services provided by a participating patient centered medical home with public health resources in local communities across the State; and\n(3) Provide complementary support for qualified individuals between office visits.\n(c) The Community Integrated Medical Home Program shall be administered jointly by the Commission and the Department.","path":["Article - Health - General"],"source_url":"https://mgaleg.maryland.gov/mgawebsite/Laws/StatuteText?article=ghg\u0026section=19-1B-02","current_through":"2026-01-01","vintage":"","retrieved_at":"2026-09-14T19:58:48Z","sha256":"3b7e9728538aa47f00eda1cce1e64d0a49484f7cd9051097bc8f81a1b3ae2e82","source_id":"us-md","stale":false,"prev":"us-md/md.-code-health-general-19-1b-01","next":"us-md/md.-code-health-general-19-1b-03"},"notice":"GroundRules: Original legal text.
