Code, Health - General § 19–320","heading":"","body":"(a) An applicant for a license shall submit an application to the Secretary.\n(b) The application:\n(1) Shall be on the form that the Secretary requires;\n(2) Shall be signed and verified as follows:\n(i) If the application is made for an individual, by the individual; or\n(ii) If the application is made for a corporation, association, or government agency, by 2 officers of the organization; and\n(3) Shall include:\n(i) The name of the applicant;\n(ii) A statement that the applicant meets the requirements of this subtitle;\n(iii) The class of the proposed hospital or related institution;\n(iv) The location of the proposed hospital or related institution;\n(v) The name of the individual who is to be the administrative head of the proposed hospital or related institution; and\n(vi) Any other information that the Secretary requires.","path":["Article - Health - General"],"source_url":"https://mgaleg.maryland.gov/mgawebsite/Laws/StatuteText?article=ghg\u0026section=19-320","current_through":"2026-01-01","vintage":"","retrieved_at":"2026-09-14T19:58:48Z","sha256":"efec33589c337b39e453a4f9cb1cba6f53a7d1b555b0dd85972098df7e270de0","source_id":"us-md","stale":false,"prev":"us-md/md.-code-health-general-19-319.2","next":"us-md/md.-code-health-general-19-321"},"notice":"GroundRules: Original legal text.
