Code, Health - General § 19–713.5","heading":"","body":"(a) If a health maintenance organization requires its subscribers to have a referral to receive consultation services in writing, the health maintenance organization shall use the uniform consultation referral form adopted by the Commissioner under § 15-120 of the Insurance Article as the sole instrument for referrals for consultation services.\n(b) A health maintenance organization may not impose as a condition of coverage any requirement to:\n(1) Modify the uniform consultation referral form; or\n(2) Submit additional consultation referral forms.\n(c) The uniform consultation referral form shall be properly completed by the health care provider referring the subscriber for consultation services.","path":["Article - Health - General"],"source_url":"https://mgaleg.maryland.gov/mgawebsite/Laws/StatuteText?article=ghg\u0026section=19-713.5","current_through":"2026-01-01","vintage":"","retrieved_at":"2026-09-14T19:58:48Z","sha256":"f2f48179a7063decee18eda007f07e2ea00c3d6c95c1ca9a540a2146b5d65b2c","source_id":"us-md","stale":false,"prev":"us-md/md.-code-health-general-19-713.4","next":"us-md/md.-code-health-general-19-713.6"},"notice":"GroundRules: Original legal text.
