Code, Health Occupations § 12–6D–05","heading":"","body":"(a) An applicant for registration shall:\n(1) Submit an application to the Board on the form that the Board requires;\n(2) Provide documentation of:\n(i) Enrollment in a doctor of pharmacy program;\n(ii) Graduation from an approved college of pharmacy; or\n(iii) For an applicant who satisfies the requirements under § 12–6D–03(a)(4) of this subtitle, proof of:\n1. Educational equivalency; and\n2.
