Donor.  The donor, if the donor is at least 18 years of age or is under 18 years of age and is:\nA.  An emancipated minor; or\nB.  Authorized under state law to apply for a driver's license because the donor is at least 16 years of age;\n2.  Agent of donor.  An agent of the donor, unless the power of attorney for health care or other record prohibits the agent from making an anatomical gift;\n3.  Parent of the donor.  A parent of the donor, if the donor is under 18 years of age and not emancipated; or\n4.  Guardian of donor.
