Plan of treatment.  A plan of treatment within 10 days of the commencement of the commitment or supervision; and\n2.  Written report.  A written report, with a copy to both the department and the individual, at least 20 days, but not more than 25 days, from the start of the commitment or supervision, setting forth the following:\nA.  The types of support or therapy groups, if any, which the individual is attending and how often the individual attends;\nB.  The type of care or treatment the individual is receiving and what future care is necessary;\nC.  Whether the individual has been cured or made noninfectious or otherwise has ceased to pose a threat to public health;\nD.  Whether continued supervision or commitment is necessary; and\nE.
