Identification.  The applicant's name, type of business organization and place of organization;\n2.  History.  The qualifications and business history of the applicant and any partner, officer or director;\n3.  Criminal and civil record.  Whether the applicant has been found guilty of any criminal offense within the past 5 years or has had a judgment of liability in a civil action involving fraud, misrepresentation or conversion. For a corporation or partnership, the application must provide the information required in this subsection for all directors, officers or partners;\n4.  Place of business.  A satisfactory report from a representative of the Secretary of State that the applicant has an established place of business at each business location in the State; and\n5.  Additional information.
