The application must include the following information:\n1.  Name and residence.  The legal name and residence and business addresses of the applicant, if the applicant is a natural person, or, if the applicant is a partnership, association, or corporation, the name of every partner, officer, or administrator of the applicant;\n2.  Location.  The location of the principal office of the applicant;\n3.  Other locations.  The complete address of any other locations at which the applicant proposes to engage in the activities regulated by this subchapter; and\n4.  Other information.
