Agreed upon medical provider.  The examination or tests must be conducted by a qualified medical provider mutually agreed upon by the chief executive officer and member claiming to be disabled.\n2.  Agreed upon place.  The examination or tests shall be conducted at a place mutually agreed upon by the chief executive officer and member claiming to be disabled.\n3.  Costs.
