Laws § 333.18113","heading":"Professional disclosure statement.","body":"Sec. 18113.\n\nA licensee shall furnish a professional disclosure statement to a prospective client before engaging in counseling services.\n\nA professional disclosure statement required under this section shall contain all of the following:\n\nThe licensee's name, business address, and telephone number.\n\nA description of the licensee's practice.\n\nA description of the education and experience of the licensee.\n\nThe licensee's counseling fee schedule.\n\nThe name, address, and telephone number of the department.\n\nThe disclosure statement shall accompany the original application for licensure.
