Laws § 339.202","heading":"Licensure or registration; application; form; fees; requirements for issuance of license or registration; expiration date.","body":"Sec. 202.\n\nAn application for licensure or registration shall be made on a form provided by the department and accompanied by the appropriate fees prescribed in article 4.
