Laws § 37.303","heading":"Voluntary identification and patch for service animal; development and availability; eligibility.","body":"Sec. 3.\n\n(1) Subject to subsection (2), the department shall develop and make available upon request a voluntary identification and patch for a service animal for a person with a disability.\n\n(2) To be eligible to receive the voluntary identification and patch described in subsection (1), the person seeking the materials shall provide all of the following:\n\n(a) An affidavit signed by the person seeking the identification and patch attesting that the service animal for which the identification and patch are being sought has been trained to be a service animal for use by a person with a disability.\n\n(b) Documentation from an appropriate health care or rehabilitation professional that the individual requires the assistance of a service animal due to a disability.","path":["MI Code","Chapter 37","Act Act-146-of-2015"],"source_url":"https://www.legislature.mi.gov/Laws/MCL?objectName=mcl-37-303","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:31Z","sha256":"9e41314b4915acb9b31d12966c8be2629609d9eb4cb53183a5deba0422cf11c6","source_id":"us-mi","stale":false,"prev":"us-mi/mich.-comp.-laws-37.301","next":"us-mi/mich.-comp.-laws-37.305"},"notice":"GroundRules: Original legal text.
