The election shall be called and conducted in accordance with section 661. The ballot shall be substantially in the following form:\n\n\"Shall the __________ mill limitation on the annual property tax previously approved by the electors of ________________________________________________________, (legal name of intermediate school district) state of Michigan, for the establishment and operation of area vocational-technical education programs be increased by __________ mills?
