Laws § 500.7012","heading":"Fees; payment and collection; service of process; designation and purpose of fees.","body":"Sec. 7012.\n\n(1) The commissioner shall collect, and the persons affected shall pay to the commissioner, the following fees:\n\n(a) Filing fee to accompany application for certificate of authority $ 200.00. (b) Certificate of authority $ 25.00. (c) Filing fee for annual statement each year $ 25.00.\n\n(a)\n\nFiling fee to accompany application for certificate of authority\n\n$\n\n200.00.\n\n(b)\n\nCertificate of authority\n\n$\n\n25.00.\n\n(c)\n\nFiling fee for annual statement each year\n\n$\n\n25.00.\n\n(2) Each MEWA shall appoint the commissioner as its resident agent for purposes of service of process.
