Pharmacies.\n(a) Each pharmacy, as defined in section 151.01, subdivision 2, shall provide the following information to a patient upon request:\n(1) the pharmacy's own usual and customary price for a prescription drug;\n(2) a record, including all transactions on record with the pharmacy both past and present, of all co-payments and other cost-sharing paid to the pharmacy by the patient for up to two years; and\n(3) the total amount of all co-payments and other cost-sharing paid to the pharmacy by the patient over the previous two years.\n(b) The information required under paragraph (a) must be readily available at no cost to the patient.","path":["INSURANCE","CHAPTER 62J.
