Definition.\n(a) For purposes of this section, the following term has the meaning given.\n(b) \"Formula\" means an amino acid-based elemental formula.\nSubd. 2. Required coverage.\nA health plan company must provide coverage for formula when formula is medically necessary.\nSubd. 3. Covered conditions.\nConditions for which formula is medically necessary include but are not limited to:\n(1) cystic fibrosis;\n(2) amino acid, organic acid, and fatty acid metabolic and malabsorption disorders;\n(3) IgE mediated allergies to food proteins;\n(4) food protein-induced enterocolitis syndrome;\n(5) eosinophilic esophagitis;\n(6) eosinophilic gastroenteritis;\n(7) eosinophilic colitis; and\n(8) mast cell activation syndrome.","path":["INSURANCE","CHAPTER 62Q.
