The work certificate shall show\n(1) The age, sex, place and date of birth and place of residence of the child;\n(2) The name and place of residence of the child's parent, guardian or custodian;\n(3) The name and address of the employer; and\n(4) The nature of the employment for which the work certificate is issued.\n2.
