The designation of essential community provider shall not be transferable to another physician, health care provider or entity.\n2. The designation of essential community provider shall be physician and site specific and shall not be effective at a site that has not been so designated pursuant to subdivision (8) of section 354.650.\n3. The essential community provider shall submit an annual affidavit to the department stating that such community provider continues to meet the qualifications for which the provider was certified.
