An applicant for a certificate shall:\n(1) Submit an application to the department of commerce and insurance; and\n(2) Pay to the department of commerce and insurance the application fee established by the department through regulation.\n2. The application shall:\n(1) Be on a form and accompanied by any reasonably related supporting documentation that the department of commerce and insurance requires; and\n(2) Be signed and verified by the applicant.\n3.
