The application must be accompanied by a fee of $25.\n(2) The application must include:\n(a) the name and address of the applicant;\n(b) the name and address of the applicant's officers;\n(c) the location, date, and time at which the applicant will conduct the casino night;\n(d) sufficient evidence concerning the structure and operation of the organization to enable the department to determine whether the applicant is a nonprofit organization; and\n(e) other relevant information requested by the department.","path":["TITLE 23.
