Stat. § 143-47.7","heading":"Notice and record of appointment required.","body":"(a)\tWithin 30 days after acceptance of appointment by a person appointed to public office, the appointing authority shall file written notice of the appointment with the Governor, the Secretary of State, the Legislative Library, the State Ethics Commission, and the State Controller. For the purposes of this section, a copy of the letter from the appointing authority, a copy of the properly executed notice of appointment as set forth in subsection (c) of this section, or a copy of the properly executed Commission of Appointment shall be sufficient to be filed if the copy contains the information required in subsection (b) of this section.\n(b)\tThe notice required by this Article shall contain the following information:\n(1)\tThe name and office of the appointing authority;\n(2)\tThe public office to which the appointment is made;\n(3)\tThe name and address of the appointee;\n(4)\tThe county of residence of the appointee;\n(5)\tThe citation to the law or other authority authorizing the appointment;\n(6)\tThe specific statutory qualification for the public office to which the appointment is made, if applicable;\n(7)\tThe name of the person the appointee replaces, if applicable;\n(8)\tThe date the term of the appointment begins; and\n(9)\tThe date the term of the appointment ends.\n(c)\tThe following form may be used to comply with the requirements of this section:\n\"NOTICE OF APPOINTMENT\nNotice is given that ______ is hereby appointed to the following public office:\nName\nPublic Office: _______________________________________________________________\nCitation to Law or Other Authority Authorizing the Appointment:\n___________________________________________________________________________\nSpecific Statutory Qualification for the Public Office, if Applicable:\n___________________________________________________________________________\nAddress of the Appointee: _____________________________________________________\n___________________________________________________________________________\n___________________________________________________________________________\nCounty of Residence of the Appointee: ___________________________________________\nDate Term of Appointment Begins: ______________________________________________\nDate Term of Appointment Ends: _______________________________________________\nName of Person the Appointee Replaces, if applicable:\n___________________________________________________________________________\n____________________\t____________________________\nDate of Appointment\tSignature\n____________________________\nOffice of Appointing Authority\nDistribution:\nGovernor\nSecretary of State\nLegislative Library\nState Ethics Commission\nState Controller\" (1979, c. 477, s. 1; 1991, c. 542, s. 8; 2003-374, s. 2; 2009-549, s. 18; 2017-6, s. 3; 2017-212, s. 8.10; 2018-146, ss. 3.1(a), (b), 6.1.)","path":["Chapter 143.
