Stat. § 90-210.132","heading":"Fees.","body":"(a)\tBy rule, the Board may set and collect fees from crematory and hydrolysis licensees, crematory and hydrolysis manager permit holders, and applicants not to exceed the following amounts:\n(1)\tLicensee application fee.\t$400.00\n(2)\tAnnual renewal fee.\t150.00\n(3)\tLate renewal fee.\t75.00\n(4)\tReinspection fee.\t150.00\n(5)\tPer cremation or reduction fee.\t10.00\n(6)\tLate fee, per cremation or reduction.\t10.00\n(7)\tLate fee, cremation or reduction report.\t75.00 per month\n(8)\tCrematory or hydrolysis manager permit application fee.\t150.00\n(9)\tAnnual crematory or hydrolysis manager permit renewal fee.\t40.00.\n(b)\tThe funds collected pursuant to this Article shall become part of the general fund of the Board.\n(c)\tNo later than the tenth day of each month, every crematory licensee and hydrolysis licensee under this Article shall remit to the Board the per cremation or reduction fees for the cremations or reductions which the crematory licensee performed during the immediately preceding calendar month. Each remittance shall be accompanied by a statement signed by an authorized representative of the licensee containing all of the following information:\n(1)\tThe name of the crematory licensee.\n(2)\tEach decedent's name.\n(3)\tDate of each cremation or reduction.\n(4)\tThe person or entity from whom each cremation or reduction was performed.\n(5)\tThe number of cremations or reductions contained in the statement.\n(6)\tThe total amount of fees remitted with the statement.
