Stat. § 90-39","heading":"Fees.","body":"In order to provide the means of carrying out and enforcing the provisions of this Article and the duties devolving upon the North Carolina State Board of Dental Examiners, it is authorized to charge and collect fees established by its rules not exceeding the following:\n(1)\tEach application for general dentistry license\t$1,200\n(2)\tEach general dentistry license renewal, which fee shall be annually fixed by the Board and not later than November 30 of each year it shall give written notice of the amount of the renewal fee to each dentist licensed to practice in this State by mailing such notice to the last address of record with the Board of each such dentist\t600.00\n(2a)\tPenalty for late renewal of any license or permit\t100.00\n(3)\tEach provisional license\t300.00\n(4)\tEach intern permit or renewal thereof\t500.00\n(5)\tEach certificate of license to a resident dentist desiring to change to another state or territory\t75.00\n(6)\tRepealed by Session Laws 1995, (Reg. Sess., 1996), c. 584, s. 1.\n(7)\tEach license to resume the practice issued to a dentist who has retired from and returned to this State\t500.00\n(8)\tEach instructor's license or renewal thereof\t500.00\n(9)\tWith each renewal of a dentistry license, an annual fee to help fund special peer review organizations for impaired dentists\t100.00\n(10)\tEach duplicate of any license, permit, or certificate issued by the Board\t75.00\n(11)\tEach office inspection for general anesthesia and parenteral sedation permits\t750.00\n(12)\tEach general anesthesia and parenteral sedation permit application or renewal of permit\t100.00\n(13)\tEach application for license by credentials\t3,000.00\n(14)\tEach application for limited volunteer dental license\t200.00\n(15)\tEach limited volunteer dental license annual renewal\t50.00.\n(1935, c. 66, s. 12; 1953, c. 564, s. 1; 1961, c. 446, s. 8; 1965, c. 163, s. 3; 1971, c. 755, s. 8; 1979, 2nd Sess., c. 1195, s. 12; 1987, c. 555, s. 1; 1993, c. 420, s. 1; 1995 (Reg.
