Stat. § 90-457","heading":"Fees.","body":"The Board may establish fees, not to exceed the following amounts:\n(1)\tApplication and an examination, one hundred dollars ($100.00).\n(2)\tIssuance of a license, five hundred dollars ($500.00).\n(3)\tRenewal of a license, three hundred dollars ($300.00).\n(4)\tRenewal of a license, an additional late fee of two hundred dollars ($200.00).\n(5)\tDuplicate license fee, twenty-five dollars ($25.00).\n(6)\tDuplicate wall certificate fee, fifty dollars ($50.00).\n(7)\tLabels for licensed acupuncturists, one hundred fifty dollars ($150.00).\n(8)\tReturned check fee, forty dollars ($40.00).\n(9)\tLicensure verification, forty dollars ($40.00).\n(10)\tName change, twenty-five dollars ($25.00).\n(11)\tContinuing education program approval fee, fifty dollars ($50.00).\n(12)\tContinuing education provider approval fee, two hundred dollars ($200.00).\n(13)\tInitial school application fee, one thousand dollars ($1,000).\n(14)\tRenewal school approval fee, seven hundred fifty dollars ($750.00).\n(15)\tInactive license renewal fee, fifty dollars ($50.00), payment due for each two-year extension.
