Stat. § 93D-7","heading":"Statements of sale.","body":"Every person fitting and selling a hearing aid, be it new or used, in the State of North Carolina, at or before the time of delivery of the hearing aid shall render to the user and/or purchaser a statement of sale to include the following:\n(1)\tDate of delivery\n(2)\tCondition of hearing aid; new, used, reconditioned\n(3)\tHearing aid identification number\n(4)\tName of manufacturer\n(5)\tPrice of hearing aid\n(6)\tCharge for fitting and service\n(7)\tName of dealer and/or fitter\n(8)\tSignature of customer.
