Code § 43-28-11","heading":"Application - Fees","body":"1.An individual seeking a license, registration, permit, credential, or endorsement related to the practice of dentistry, shall apply to the executive director on forms prescribed by the board.\n2.The application must:\na.Be verified under oath that all statements contained in the application are true to the applicant's knowledge;\nb.Contain a recent autographed picture of the applicant; and\nc.Include the application fee established by the board.\n3.The board may, by rule, create or modify fees associated with any license, registration, permit, credential, or renewal.
