Stat. § 44-32,129","heading":"Group and individual contracts; required provisions.","body":"Every group and individual contract holder shall be entitled to a group or individual contract. The contract shall not contain provisions or statements which are unjust, unfair, inequitable, misleading, or deceptive or which encourage misrepresentation as described by section 44-1525. The contract shall contain a clear statement of the following:\n(1) Name and address of the health maintenance organization;\n(2) Eligibility requirements;\n(3) Benefits and services within the service area;\n(4) Emergency care benefits and services;\n(5) Out-of-area benefits and services, if any;\n(6) Copayments, deductibles, or other out-of-pocket expenses;\n(7) Limitations and exclusions;\n(8) Enrollee termination;\n(9) Enrollee reinstatement, if any;\n(10) Claims procedures;\n(11) Enrollee grievance procedures;\n(12) Continuation of coverage;\n(13) Conversion;\n(14) Extension of benefits, if any;\n(15) Coordination of benefits, if applicable;\n(16) Subrogation, if any;\n(17) Description of the service area;\n(18) Entire contract provision;\n(19) Term of coverage;\n(20) Cancellation of group or individual contract holder;\n(21) Renewal;\n(22) Reinstatement of group or individual contract holder, if any;\n(23) Grace period; and\n(24) Conformity with state law.\nAn evidence of coverage may be filed as part of the group contract to describe the provisions required in subdivisions (1) through (17) of this section.","path":["Chapter 44 - INSURANCE"],"source_url":"https://nebraskalegislature.gov/laws/statutes.php?statute=44-32,129","current_through":"Laws 2026","vintage":"","retrieved_at":"2026-09-17T21:01:11Z","sha256":"5e80117b581ab43924e0112106826a473fc9555b5ac63b8202ffbeb1760f9f29","source_id":"us-ne","stale":false,"prev":"us-ne/neb.-rev.-stat.-44-32-128","next":"us-ne/neb.-rev.-stat.-44-32-130"},"notice":"GroundRules: Original legal text.
